Managing Director of precast concrete supplier Sateba resigns after amid HS2 Chiltern tunnel quality concerns
In August at HS2's 2,5km long Chitern Tunnel, some quality problems had been discovered with the precast concrete supplied by Stanton Precast (Sateba). The MD of the company Ian Steel had got the job just 7 months ago on 8 February.

Maybe they used "Fiber Reinforced Polymer and Macro Synthetic Fiber" ın segments instead of traditional steel?
There was such a case in İzmir Metro in Türkiye a few years ago:
